This form seeks your consent for Guide Dogs to capture, use and publish images or information about you (or a minor in your care). This may include photographs, audio and video recordings, details, quotes and text. In this consent form, “Guide Dogs”, “we”, “us” or “our” means the members from time to time of Royal Guide Dogs Australia, including Guide Dogs NSW/ACT.
